We are a community health center. By providing primary care and counseling on site and coordinating a variety of support services, our goal is your complete wellness. If you are uninsured, our team can help you determine what insurance options are right for you. By accepting Healthy Michigan, Medicaid, Medicare, private insurance, and offering an income-based sliding fee scale for the uninsured, we make healthcare accessible to every person in your family.

We have the same phone number, same providers and staff, same services and a lab, a BATA stop out front.
Our Hall Street and Logan Valley offices are now permanently closed as we become ONE new location.
What We Do
We believe health should be simple. We know healthcare is confusing. We can help! We will help you determine what insurance options are right for you. If you are uninsured, we offer a sliding fee scale. No one is turned away.
We treat our patients like people, not symptoms. Our doctors, nurse practitioners, and staff are with us because they are genuinely passionate about community health. From routine check-ups, diagnosing and treating illness, vaccinations, screening tests, and more, we take a proactive, personal approach to healthcare.
Our goal is your complete wellness. For patients who have established care with one of our providers, we offer a variety of support services such as on-site counseling, translation services, transportation assistance and help accessing dental care, if needed.
